Physical Activity Pathways Project

The HSE Health and Wellbeing Physical Activity Pathways Project aims to create an online solution for people with chronic diseases to be physically active in their communities. This is one component of the Physical Activity Pathways in Healthcare Model (PAPHM).

The project's objective is to provide:

  • clients with access to digital resources to engage in physical activity for health benefits and chronic disease prevention
  • health and social care professionals with access to a virtual service to support clients in clinically-safe physical activity

Routine care visits will include discussions about physical activity, following the 'making every contact count' policy. Clients will be encouraged to register and participate with the programme. Upon registration, each client will go through an electronic triage that includes calculation of activity readiness. They will then be given suitable options that range from:

  • access to online pre-recorded exercise programmes such as pilates or strength and conditioning
  • access to online supported behavioural change programmes (these may be virtual live sessions)
  • signposting to local activities
