General IT devices
Complete and return the IT equipment request form to your service desk mailbox if you need a:
- desktop PC
- laptop
- docking station
- monitor
- keyboard
- mouse
You must include the relevant cost or budget code on the form with sign-off from your manager.
Complete the form in Word. Print and sign the form and submit it through your self-service portal or scan it and email to your service desk mailbox.
USB memory stick
Only HSE-approved USB memory sticks, distributed by the ICT directorate, may be used to store or transfer HSE data.
To request a USB memory stick you must complete the USB memory stick form and email it to your service desk mailbox.
Tablets
To order a tablet, you must complete the electronic device request form. The form must be completed by the requesting user and budget holder and sent to ict.tablets@hse.ie
Smartphone and MiFi devices
Smartphone and MiFi requests are processed by your local mobile administrator. Do not send your request form to the National Service Desk mailbox as it will not get processed.
Using the relevant request form, you can:
- order a new smartphone
- upgrade your phone
- reallocate your phone
Ordering or upgrading to a new smartphone
You will need to complete the smartphone request form in Word. Print and sign the form, scan it, and email to your local mobile administrator. Incomplete forms or handwritten forms without signatures will not be processed.

Reallocating your smartphone to another user
To reallocate your smartphone to another user you must complete the smartphone reallocation form in Word. Print and sign the form, scan it, and email to your local mobile administrator. Incomplete forms or handwritten forms without signatures will not be processed.
The form is completed by you, your line manager and general manager.

Setting up international voice and data roaming
To use your HSE phone while abroad you must have international voice and data roaming enabled. To get this enabled, complete the smartphone roaming request form and submit it to your local mobile administrator.
The form is completed by you and authorised by your line manager.

Software requests
You must purchase a licence for software that requires a licence.
You will need an approved purchase order (PO) to send to the vendor to get an order raised.
The vendor will provide proof of purchase and/or licence details. You need to include this as part of your request to the NSD (include it with the software request form).
To request software installation you will need to complete the software request form
The form must be completed by you, your senior manager or budget holder.
You must complete the form in Word. Print and sign the form and email it to your service desk mailbox.
Getting access to blocked internet content
To get access to blocked internet content you must complete the Internet content filter exemption form. The request must be approved by your line manager (at General Manager level).
Submit the completed form to your service desk mailbox.
